Can anyone tell me how these things are activated? Do you have to
show ID, give name and address and so forth?

Depends in which country you are referring to.

I bought one (pay as you go sim) in China day before yesterday, no questions
asked,

In Thailand, all sim cards are now required to be registered.

Sorry, doesn't ring a bell.

In Malaysia they're sold over the counter for cash without any registration
process or ID check.

I recently bought one in Thailand and the registration consisted of someone
copying my name off a photocopy of my passport in such a way that nobody
would ever have been able to read it (maybe three of the letters were
correct). Not too rigorous.

You are in the USA. Are you asking about payg in the USA or about using
it in another country? Most countries (USA included) do not ask for any
information. You can even buy payg sims on eBay.
