Samsung Evo 64gb SD card corrupted, won't format

Hi everyone I'm not sure I'm posting this is the right place. But I have a Samsung Evo 64gb micro sdxc UHS-1 that got corrupted a couple weeks ago. I first noticed something was going on when my phone quit reading the SD card. After a day of it being able to be read\ not read it completely stopped reading it. I tried formatting it in my phone (which is a Samsung Galaxy S7) and it got stuck at 20%. So I took it out and put it in my tablet. When I tried formatting it itd get stuck at 20% for about 15 seconds. It then popped up with a message saying "command' 19 volume partition disk:179, 64 public' failed with 400 26 command' failed. After doing it a couple times I decided to put it in my computer. And my computer wouldn't read it either. Is there anyway to fix this? Or is my SD card toast??
