how to restore sd card after formatted

Accidentally formatted 512 gb sd card from my phone and now it wont read and it keeps freezing my phone

You probably formatted it in NTFS files system, which is fine for Windows but not so good for Linux/Android. Forget the data that were on it and just re-format it in FAT32.
