FPS Low stuck on 135ghz gpu

I've been trying to figure out for a long time why my FPS Is low. appx 5-10fps on minumum graphics (15-25 in private instance with no animations). I've tested out my fps with FurMark (stress test) and it can sustain 20fps max gpu
Runescape seems to use 5-20% of my gpu for some reason.
The fps drops drastically when in combat, or during any type of attack animation.

I've tried:
Setting RS to use nvidia through the nvidia control panel
updating drivers + downgrading drivers to see if newer ones are causing it
updated bios
all things on performance mode
switching rs settings between opengl and directx
making sure rs is on minimul graphics
adjusting ram allocation
cleaned internal components, and replaced thermal paste
completely reset laptop to factory, and only updated necessities

I've also tried overclocking it however that doesnt make any difference. While doing that I've noticed the GPU seems to drastically lower when the game is loaded (stuck at 135ghz). It uses 745ghz while loading, but when it finishes loading - it drops to the 135 and doesn't change regardless of any ingame changes, overclocked or not, etc.

Nothing seems to be working here. I've also tried switching to intel graphics to see if it would be better, but the nvidia is 1-2 fps more then it. It doesn't seem like that should be happening. Any ideas?
