I have a 24" Vizio Smart TV with one HDMI port hooked up to an old Toshiba HD DVD through HDMI & the DVD is hooked up through an optical cable to an old Niro Reference Amp & speakers. DVD sound is good. But I need to output sound from the TV antenna and the smart apps like Netflix into the Niro amp.

If I get a HDMI splitter box with 4 HDMI inputs & 1 HDMI & 1 Optical output, will that give me quality sound from the antenna & Smart Apps instead of using the puny internal speakers on my TV? Or can I run a composite to optical cable from the TV to the Niro Amp?

No optical out. The TV has a headphone jack. And composite and component connectors. The Niro amp has 2 digital optical & 1 coaxial and RCA plug analog inputs. I have my DVD plugged into one of the optical inputs.

As Alabalcho says the HDMI switch/audio extractor won't help because you are trying to get audio from the TV smart functions which are internal.
If the TV has HDMI ARC then you can use an extractor like this one
http://www.amazon.com/ViewHD-Extractor-Support-Converter-VHD-H2HARC/dp/B00LKF6CPG
If not then you can convert the headphone out of the TV to optical audio with one of these
http://www.amazon.com/SANOXY%C2%AE-Analog-Digital-Converter-Adapter/dp/B005F20756/ref=pd_sim_23_1?ie=UTF8&refRID=0HT2H5S2J8KHB40MQ7FW
You would need to set the volume of the TV low and bring it up slowly as you could overload the input of the converter and cause a lot of distortion.
