Need label printing software for serial numbers

I'm looking for a preferrably open source software that will perform the following:
Take a text file of MAC numbers without the colons or decimals separated by a <CR>
Insert the colons or decimals
Insert the numbers on a second line under a company name.

We use a network device called a rabbit and we purchase them from Digikey. When they come in we scan the MAC off each rabbit board into a text file. We use Avery 5167 4x20 labels to print our company name first line and the MAC second line. We've been using software no longer in production and the PC it was on had a catastrophic hard disk failure so I'm trying to replace a software that's no longer sold as the company is out of business. Any suggestions? Thanks in advance.

Why not create a table in e.g. Word with each cell containing your Company Name, and empty second row. When you scan the barcode, use TAB, Right Arrow to move to the empty row, and insert the barcode there. When you have full page of labels, print them on ordinary printer.
